Referrals from additional sources may be considered if the individual otherwise meets program criteria, most referrals to this program come from CIT Officers. Now this program provides clinical intervention solutions to people that are being released from inpatient psychiatric stays at Erie County medic Center and Buffalo Psychiatric Center. While outpatient service providers, the team works in collaboration with discharge planners. Now let me tell you something. While they have always been transitioning to ongoing outpatient maintenance, people referred to this program will get quite short term clinical intervention and peer outsourcing from a team of MTS Counselors and Peer Counselors. However, referrals to this program usually can solely be made by discharge planners at these 3 hospitals, through an arranged referral process. Now this goal service is to keep guys and girls from presenting at regional psychiatric emergency rooms for hospital ‘readmission’, and to see that these people correctly transition back to community and are always securely connected with appropriate outpatient solutions identified in the discharge plan.
